This application is for businesses or individuals whose CWHBA membership has lapsed and who are seeking to renew and return to active status.
Completion of this form allows us to:
- Confirm current business and contact information
- Verify eligibility for CWHBA programs that require active membership
- Align records across our systems for accurate renewal processing
Submission of this application does not automatically reinstate membership. Membership status will be restored upon completion of renewal requirements, including payment of any outstanding dues.
